Charlie’s Process 

 

Initial Consultation

Meet Charlie. Charlie comes in with his mum to discuss his physical needs, his goals, and the services and supports currently being used. We provide information about GYFT, our process, the expected outcome and the timeframe – so we’re all on the same page.

Step 1: Assessment

We gather all of the information we can, interviewing Charlie and his mum, as well as any support workers and services. We collect all of Charlie’s previous assessments and reports, to understand his abilities, needs and goals.

Step 2: Customised Planning

Together, we create Charlie’s personalised program – including his complete profile and a life design plan, created to help him build his skillset and work towards his goals.

Step 3: Occupational Coaching

With one-on-one coaching, we work with Charlie to build his independence and discover what’s possible, all while keeping any environmental or support requirements in mind. This might include coaching Charlie to participate in new activities, learning and education, as well as working and volunteering.

Step 4: Training and Development

We develop a set of resources for Charlie and his team to help him continue to learn and build his skillset. We also bring in specialist practitioners and train his carers to become specialists in their own right. This includes understanding how to pace new milestones, as well as transition planning, continuity planning, specific training and education resources and on-boarding of new staff.

Step 5: Coordination

Via one-on-one training for Charlie’s carer, his mum, we help her to implement the continuity plan, step down her involvement and level of care for Charlie, and ultimately prevent burnout. She’ll begin to manage the broader support system with clear structural and feedback mechanisms in place.
