Traditional health and support services often exist in silos – not talking to each other or sharing information about your plans, your progress or your goals. Here at Get Your Future Together, we prefer a more holistic approach. Mental health, employment, rehabilitation, education and wellbeing – we tackle the lot. We’ll even help to find gaps in your NDIS support, government funding or extra disability care, so you can more effectively use the funding that’s available to you. Because after more than two decades in mental health and occupational therapy, we know that the best way to create meaningful engagement, productivity and purpose is through a unified support network that acts as one. Whether it’s school, therapy groups, family caregivers, doctors or specialists, everyone has their own part to play in providing the care and support you need. Silos, be gone.

Supporting Your Support Network

Having a healthy support network around you can make a big difference. But like you, it’s important that they have support of their own. So in addition to creating a personalised game plan for you, we’ll also work closely with your primary carers to figure out the best way they can help you, too. We’ll bring them up to speed with your goals, and coach them on the things they can do to help you reach them. Whether that’s giving them the tools to help cultivate greater independence and autonomy for you, or encouraging them to use self-care techniques to look after themselves as well. The more support your carers need, the more we’ll give them. Because the happier and healthier your support network is, the better off you’ll be.Our Process

Based on a 360-degree understanding of your needs, our holistic process helps you develop your goals and work towards them, alongside your support network. While there’s a different process for those wanting help with career development and those living with a disability, both have the same end goal: helping you get your future together, on your own terms.
